FAIZABAD, AFGHANISTAN, AUGUST 2008.Terribly scared and in tears, forty year old Sangimo lies on the operating table at Faizabad Maternity Hospital. Arriving with breathing difficulties, serious bleeding and in great pain, it took Sangimo 12 hours to travel by jeep from her home in Ishkosham village. At four months pregnant, Sangimo has already experienced four previous miscarriages. Having already tried opium to relieve the pain and Zamj (a local salt rememdy that is used internally) to stop the bleeding, her husband finally brought her on the long journay to Faizabad in fear of her life- his first wife died during childbirth. For girls in this remote regoin of Afghanistan, it is shameful to ask their mothers for advice on health or sex issues. The protocol is that they should keep quiet even if they are in pain. She has never seen a birth before before, "Sometimes I'm afraid but I don't know why. Always I'm thinking will I live or die". During her pregnancy, Sahliha has turned to her religion for comfor praying constantly to Allah, "I ask that I will deliver successfully, I will get well as soon as possible and hat my baby has a long life". SANDARA, AFGHANISTAN, AUGUST 2008.Twelve year old Siamo rummages through a box looking for thread so that she can complete the embriodery that she will sell to raise money for her adopted family..After her mother died in childbirth at just twenty-five years old during the delivery of twins one winter, Siamo and her sister were adopted by their aunt Sabrima. Having never attended school, Siamo spends her days sewing and weaving hankerchiefs or rugs. Last month she was sold for a Jahez, or bride price, of 20,000 Afghanis. Worried about having children herself, she hopes that she can wait a little "My future husband is away in Iran just now. I will see what happens"..Girls such a Siamo have nowhere to turn for advice on health or sex issues. SANDARA, AFGHANISTAN, AUGUST 2008.Her face decorated with make-up and wrapped in hand embriodered swaddling clothes, 20 day old Hajira is cradled by her mother Jowhar-Bibi at their home in the remote Sandara village..Two months before the birth, mother of four Jowhar-Bibi became extremely sick. Rushed to the nearest clinic back of a stranger's motorbike after she developed complications during childbirth, she was petrified that she would have to give birth on the road- an event that would bring great shame on a family in Afghanistan. After an uncomfortable journey, Hajira was born assisted by a midwife at the Khourdakon health post. "I didn't even realise that I had delivered because of the pain killers. When my child was crying I couldn't believe it was my child... that I had delivered her". Jowhar-Bibi has been quick to chat to all her neighbours about her experience of child birth- she is the only woman in the village to have accessed medical care in an area where traditional birth attendents and medicines are the norm. During freezing conditions, she spent seven days in pain. "She was crying and making a lot of noise... There was a lot of bleeding. A woman came over to help us but she coudn't do a lot. She wasn't a midwife but when we have a problem like this, then she's the best support we have"..Gulzaman prayed to keep his wife alive but with the mountain passes blocked with snow, there was no chance of any help arriving..Their daughter is now withdrawn since the traumatic death of her mother and cries all the time. "She was feeling unwell and in a lot of pain. There was a lot of bleeding but there was nothing I could do- I just had to stand by". Despite finding transport across the the mountains for the seven hour journey to Faizabad Hospital, by the time they reached the city there was nothing that could be done to save her. Fahima's father disowned her, as is the case with children who are orphaned due to maternal mortality- Zebanisa was the only person who stepped in to help. She survives on charity from neighbours.
